Roosevelt Vision Clinic
At Roosevelt Vision Clinic, we strive to provide comprehensive, primary eye care for the whole family. Preventative and routine eye exams are important to maintaining good eye health. Often, eye and vision problems do not have obvious symptoms or signs, but are easily diagnosed by a licensed optometrist. By diagnosing eye and vision conditions early on, our optometrist is able provide treatment options and in many cases restore or prevent vision loss.
Our Doctors
Dr. Jon Wilson
Dr. Jon Wilson was raised in Neola, Utah graduating from Union High. He graduated from University of Utah with a bachelor's degree in Biology. Jon obtained his Doctor of Optometry and Masters in Education, Visual Function in Learning from Pacific University in Forest Grove, Oregon. He completed an internship at the Eye Institute of Utah, specializing in LASIK and cataract care with an added emphasis on ocular disease.
Jon loves serving humanitarian eye care missions and currently is the director of the Hope Arising Eye Clinic in Dera, Ethiopia. He joined Roosevelt Vision Clinic in 2001 and enjoys being a part of the Uintah Basin community. Jon spends his spare time with his wife Heather and five children pursuing snowboarding, traveling, antique cars, a good parade, and anything outdoors. |
Dr. Jason Kowallis

Dr. Jason Kowallis was raised in Roosevelt, Utah graduating from Union High. He graduated from Utah State University with a bachelor's degree in Biology. Jason and his family moved to Memphis, TN to obtain his Doctor of Optometry from Southern College of Optometry graduating Cum Laude. While on internships he was able to further his education by working at Family Optometry in Chandler Arizona specializing in Vision Therapy. Other internships were done at the VA Hospital in SLC, Utah and Moran Eye Center where he worked with ocular disease and contact lens. Upon graduation Jason joined Roosevelt Vision Clinic in May of 2012. In April of 2016 he received his fellowship from the College of Optometrists in Vision Development making him 1 of 4 optometrists in the state of Utah to receive such specialty.
Jason lives in Roosevelt with his wife Christina and their four children. He enjoys spending time with his family and enjoying all there is to do in the Uintah Basin. |
Dr. Cade Kowallis
Dr. Cade Kowallis was raised in Roosevelt Utah. He graduated from Union High school and went on to receive is bachelor degree from Utah State University in biology. After Utah State University he went to Southern College of Optometry to receive his degree in Optometry. While going to Optometry school he did summer research on Vision Therapy with Dr. Paul Harris and Dr. Marc Taub. He also did Internships to further expand his knowledge at the VA hospital and Moran Eye Center in Salt Lake City working with ocular disease and contact lenses. Also an internship at Child and Family Eyecare in Pleasant Grove Utah working especially with Vision Therapy for children and Traumatic Brain Injuries. Upon Completion of School Dr. Cade Kowallis joined Roosevelt Vision Clinic in May of 2015.
Cade currently lives in Roosevelt with his wife Kelsie and three kids. He has enjoyed being back in the basin with the community that he grew up in. |
